 Join Your Home Medical Care 

* A 6 month commitment to be a member of Your Home Medical Care.

 

* Once you join and schedule your first visit, we will come your home and see all family members. We can draw necessary bloodwork and order necessary tests at the first visit.

 

* If you require Urgent Care, first purchase a "One-Time Consult". Because if you join the family practice long-term, we require a face-to-face appointment before calling in medications for you.

* After your first face-to-face appointment, we will handle emergent health issues in your family on an as-needed basis, employing phone or facetime or home visits whenever possible. This will be done at no extra charge.

​

* We will call in any medicines you need, so you will only need to go to the pharmacy once.

​

* All appointments are conveniently booked by the patient online.

​

* Payment will be through monthly credit card withdrawal. The price depends on the size of the family living in the home. We designed it this way to encourage families to let their elderly loved ones come live with them, which may be much more work for us, but is cost-effective for you and convenient and beneficial for your elderly loved ones.

* You have to remain a member for at least 6 months before you can cancel membership.

 

* Keep in mind, the cost for your whole family for a year of our care is roughly equivalent to the cost of one E.R. visit to stitch a laceration or get an antibiotic for pneumonia. We will take care of your whole family for almost all of your acute and chronic medical problems in YOUR Home, including lacerations, minor surgeries, telemedicine, well-visits, sports physicals, weight loss management, needed consults with specialists, lab and X-ray orders, etc.

* Once you JOIN our Family Practice, you will print out and fill out a document and schedule your first appointment.
